In the beginning, he has difficulties working at the hospital, as they say to him "it's like a garden, we give them food and water". He thinks it's hard that they aren't doing anything to help them.
He wants to figure out what is wrong with these people, and he begins to read all their journals and figures out that they all have something in common. He is very keen to help these people.
We see that clearly, when he figures out the L-DOPA is working, he hosts an event for some rich people, to earn money to give them all the medicine. He does that even though he isn't good around people.
Dr. Sayer is in the start not very good around people. When one of his new colleagues asks him if he wants to get together after work. He says that he has other plans.
We later see that these plans are playing his piano alone. He has never been married or with a woman, because of his social awkwardness and weird interests.
Another example of him being socially awkward is when he takes his patients on a trip. They end up at a party, where all the patients and caretakers dance, in contrast to Dr. Sayer who sits in a chair.
