The text is chronologically, because the story goes from start to end. But there is a flashback where they talk about Jack's cousin who is gay.
The text takes place in the present and is clearly taking place in the United States. Thanksgiving is typically the American family gathering point, football is also American as football we know is pronounced soccer. The family names also seem typically American; Jack - Brett - Ben - Ruth, etc.
The environment seems like it is quite normal American middle class, where we get the perception that Jack's parents are alcoholic.
